Specific information necessary to develop preliminary options is included in the following attachments:

• Company info sheet – please complete and issue to us for initial review of options.
• Census – note that there are two different formats included: please complete the ‘medical only’ tab for health insurance quotes, and please complete the ‘all lines’ tab when interested in evaluating additional options for your firm.

Specific steps:

1. Review info sheet and census.
2. Complete as much info as possible and issue to Hartman, either by fax or email.
3. A Hartman consultant will respond within 24 hours to confirm receipt, identify questions, and determine “next steps”.
   
   1. Note that a “best practice” is to begin this process in excess of sixty-days prior to the effective date, so as to provide sufficient time to develop full understanding of circumstances, goals, and a thorough analysis.
